This is part of dignity. Feedback mechanisms should be understandable, proportionate and accessible to the people the organization serves. When the process is safe, the institution receives better information about the real experience of its programs.

Learning should distinguish activity from benefit

Counting sessions, participants or distributed resources can show activity, but those figures do not always explain whether the intended benefit occurred.
